ResCare HomeCare is seeking RN Case Managers to be responsible for coordination of care and treatment to our home bound patients. The RN will be responsible for SOC/Recertification/Resumptions/Transfers/ and Discharges. In addition, the RN will identigy the needs of their patients and work with the interdisciplinary team to deliver individualizd treatment plans.
Minimum of one year RN experience in home health care prefered. Knowledge of Medicare/Medicaid regulations and Oasis training a plus.
